29 CFR 1910.334(a)(3)(ii): Attachment plugs and receptacles were connected or altered in a manner which prevented proper continuity of the electric equipment grounding conductor at the point where plugs were attached to receptacles: a) Wheel polishing area - On or about August 8, 2017 and at times prior, employees were exposed to electrical hazards in that the plug for a portable pedestal fan had its grounding prong removed.
