29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a) Jobsite: On December 22, 2016 and at times prior thereto an employee performed framing activities on the exterior top plate.  The employee straddled the top plate without a fall protection system in place.    b) Jobsite: On December 22, 2016 and at times prior thereto employees performed framing activities on the second floor with open sided floors.  Guardrails were not in place at the front or back of the second floor.     Note) Certification of corrective action is required for this item.

No action was taken to correct or minimize the hazard.  b) Jobsite:  On December 21, 2016 and at times prior thereto employees did not recognize or acknowledge the hazard of working near an open sided floor. No action was taken to correct or minimize the hazard.    Note) Certification of corrective action IS required for this item.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways that would enable each employee to recognize hazards related to ladders and stairways, and train each employee in the procedures to be followed to minimize these hazards:  a) Jobsite: On December 21, 2016 and at times prior thereto an employee used a self-supporting ladder that was not tall enough to safely position the employee for the work being done.  The employee did not recognize or acknowledge the hazard of working from the top cap of a self-supporting ladder.  Actions were not taken to minimize or correct the hazard.
