29 CFR 1910.22(a)(3): Employer failed to ensure walking-working surfaces are maintained free of hazards:    On or about January 6, 2017 an employee tripped on a loose floor mat on an elevated platform adjacent to the Autoline machine with a shear table less than 4 feet high and, as a result, fell onto the Autoline shear table and had his fingers crushed under the machines hold-downs.
