29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about February 15, 2017 at or near 4 Auriel Court, Maumelle, AR 72113, the employer exposed employees placing a ladder leaned against the roof, to the ground, to struck-by hazards.  Employees were not provided hardhats to protect their heads from falling objects that could cause physical harm.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about February 15, 2017 at or near 4 Auriel Court, Maumelle, AR 72113, the employer exposed employees working at approximately 12 feet above lower levels to fall from height hazards. Employees were working on a 10/12 pitched roof without any type of fall protection.    The Nunez Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1102570, citation number 1, item number 1 and was affirmed as a final order on January 21, 2016, with respect to a workplace located at 431 Lake Hamilton Drive, Hot Springs, AR 71913.
