This condition exposed employees to struck-by hazards.

At or about March 24, 2017 and times prior thereto, at the worksite located at or near 111 Frecourte Cove in Maumelle, Arkansas, employees were observed operating pneumatic nail-guns without any eye protection. This condition exposed employees to struck-by hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    At or about March 24, 2017 and times prior thereto, at the site located at or near 111 Frecourte Cove in Maumelle, Arkansas, employees were observed using a portable ladder to access a roof, and the ladder did not extend at least 3 feet beyond the roof's edge. This condition exposed employees to fall from heights hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about March 24 and times prior thereto, at the worksite located at or near 111 Frecourte Cove in Maumelle, Arkansas, employees were observed replacing shingles on a residential house over 6 feet without any fall protection. This condition exposed employees to fall from height hazards.       The Nunez Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1102570, citation number 1, item number 1, and was affirmed as a final order on January 27, 2017, with respect to a workplace located at 431 Lake Hamilton Drive, Hot Springs, AR 71913.
