29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with the standards set forth in 29 CFR 1926 as evidenced by the violation(s) noted during the course of the inspection:    A)    Precision Cut Carpentry, LLC, conducting work 7604 Shoemaker Drive, Waterville, OH 43566: On or about March 21, 2017, the employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as, but not limited to, falls during job site operations.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:     A)    Precision Cut Carpentry, LLC, conducting work 7604 Shoemaker Drive, Waterville, OH 43566: On or about March 21, 2017, the employer did not ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems, or alternative measures of fall protection while installing house fascia board and sheathing 7/12 pitch roof of the two story residential building. Employees did not use fall protection and were exposed to approximately 11 feet fall hazard from the roof to the ground below.

Employees were exposed to potential impact injuries caused by falling materials and/or equipment.
