29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:    Precision Cut Carpentry LLC., on or about November 01, 2019, at the address of 24962 Saddlehorn Drive Lot 308, Perrysburg, OH: The employer did not ensure that the scaffolding platform, supported by the CAT TL 642 extending boom forklift, from which the employees were located was attached and secured to the supporting forks. The employees, using the lift to access the roof, was exposed to an approximately 18 foot fall hazard from the platform to the ground below.

Such programs would support the employer and employees in identifying and avoiding potential hazards such as, but not limited to, falls during job site operations.     Precision Cut Carpentry LLC., was previously cited for a violation of this Occupational Safety and Health Standard (29 CFR 1926.20(b)(1)), which was contained in OSHA inspection #1219150, Citation number 1, Item number 1a, and was affirmed as final order on 07/17/2017, with respect to a workplace located at 7604 Shoemaker Drive, Waterville, OH 43566.

Employees were observed working on the residential roof without the benefit of fall protection training from Precision Cut Carpentry.     Precision Cut Carpentry LLC., was previously cited for a violation of this Occupational Safety and Health Standard (29 CFR 1926.503(a)(1)), which was contained in OSHA inspection #1219150, Citation number 1, Item number 1b, and was affirmed as final order on 07/17/2017, with respect to a workplace located at 7604 Shoemaker Drive, Waterville, OH 43566.
