29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment:    a. On or about April 3, 2017, in the pulping and purification building on the 4th floor at washer W-11 during the conduct of an experimental trial where steam was injected to heat the water going to the repulper dilution and shower header within washer W-11 the employer had not assessed the thermal burn hazard at the washer to employees and the need for personal protective equipment, exposing employees to thermal burn hazards from the heated water, pulp material and/or steam.
