29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition  a.  #1 Digester area;  On or about January 29, 2022, the employer exposed employees working in the digester area to fall hazards and burn hazards, in that the employer failed to maintain an adequate floor covering at the U-drain that was used for hot condensate coming from a mill liquor pre-heater.
