29 CFR 1910.151(c): Where the eyes of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes were not available for immediate emergency use:    Employees working in the tire shop were exposed to corrosive burns to the eyes in that an adequate emergency eyewash was not available.  Employees used corrosive chemicals such as Red Devil degreaser to clean wheels.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours:  On or about June 1, 2017 - An employee was injured as a result of a work related incident requiring a hospital stay and was not reported to OSHA until June 5, 2017.
