29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about June 19, 2017, employees were using pneumatic nail guns to apply roofing materials at the residential apartment complex under construction without using eye protection, exposing employees to the hazard of being struck by flying objects.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction 6 feet (1.8 m) or more above lower levels were not protected from falling by the use of guardrail systems, safety net systems or personal fall arrest systems nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented: On or about June 19, 2017, employees were roofing the residential apartments under construction up to approximately 32 feet high without fall protection, exposing employees to a fall hazard.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access: On or about June 19, 2017, employees were roofing the residential apartments under construction approximately 32 feet high using an extension ladder to access the roof that did not extend at least 3 feet above the upper landing surface, exposing the employees to a fall hazard.
