29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.      On or about June 20, 2017, at the trenching site located at the Polecat Creek Bridge on S. 49th West Avenue in Sapulpa, OK, employees were allowed to use a ladder that did not extend at least 3 feet beyond upper level.  The employees were exposed to the hazard of falls.

29 CFR 1926.100(a):  Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:      On or about June 20, 2017, at the trenching site located at the Polecat Creek Bridge on S. 49th West Avenue in Sapulpa, OK, employees did not wear protective helmets while working in a trench more than 20 feet deep, exposing the employee to being struck by falling/collapsing soil.      Cherokee Pride, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.100(a), which was contained in OSHA inspection number 1206000, citation number 1, item number 1a, and was affirmed as a final order on May 22, 2017, with a final abatement date of April 13, 2017, with respect to a workplace hazard at W. 81st Street and 23rd West Ave. Tulsa, OK 73132.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:      On or about June 20, 2017, at the trenching site located at the Polecat Creek Bridge on S. 49th West Avenue in Sapulpa, OK employees were allowed to work in a trench more than 4 feet deep that had a ladder in place which was not used.  Employees travelled more than 50 feet to exit the trench.  The employees were exposed to the hazard of being struck by falling/collapsing soil.      Cherokee Pride, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.651(c)(2), which was contained in OSHA inspection number 1206000, citation number 1, item number 2, and was affirmed as a final order on May 22, 2017, with a final abatement date of April 13, 2017, with respect to a workplace hazard at W. 81st Street and 23rd West Ave. Tulsa, OK 73132.

On or about June 20, 2017, at the trenching site located at the Polecat Creek Bridge on S. 49th West Avenue in Sapulpa, OK, no protective devices were in place to protect employees working in a trench that was more than 5 feet in depth.  The employees were exposed to the hazard of being crushed by fall soil.         Cherokee Pride, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1206000, citation number 1, item number 5b, and was affirmed as a final order on May 22, 2017, with a final abatement date of April 13, 2017, with respect to a workplace hazard at W. 81st Street and 23rd West Ave. Tulsa, OK 73132.
