29 CFR 1926.502(d)(6)(i): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any object which is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself: A snaphook of an integrated lanyard of a Guardian Personal Fall Protection System was attached directly to the webbing the same lanyard. This lanyard was used to protect an employee from falling approximately 50 feet from an unprotected platform of a P Series Hydro Mobile Scaffold.

29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee: An employee attached a 4 foot long lanyard on the end section of a guardrail system on a 50 foot high work platform of a Hydro Mobile Scaffold. The anchor point was not rated to support at least 5,000 pounds.
