29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not fit tested prior to initial use or annually thereafter:   At the St. Boniface Catholic Cathedral located at 110 Buchanan Street, Edwardsville, Illinois, 62025, employees were required to wear N95 respirators during tuck pointing operations. The employer did not ensure that employees were fit-tested to the N95 respirator prior to initial use or annually thereafter.
