29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: A) Nox US, LLC, located at 931 South Springville Ave, Fostoria, OH 44830: On or about July 10, 2017, the employer did not ensure that employees were protected from fall hazards while they were walking and working on the I-beams, located around the hopper of the PVC Recycle System on Line 1, in order to conduct maintenance activities such as, but not limited to, cleaning the hopper. Employees were not protected by methods such as, but not limited to, guardrails or safety net systems or by personal fall arrest systems and were exposed to fall hazards of up to 40 feet from the I-beams to the floor below.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: A) Nox US, LLC, located at 931 South Springville Ave, Fostoria, OH 44830: On or about July 11, 2017, the employer did not document and utilize specific procedures to control hazardous energy of the equipment such as, but not limited to, the hoppers butterfly valves of the PVC Recycle System on Line 1. Employees did not control all hazardous energy sources such as, but not limited to, pneumatic before performing activities such as, but not limited to, unplugging and cleaning the hopper. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work. B) Nox US, LLC, located at 931 South Springville Ave, Fostoria, OH 44830: On or about August 4, 2017, the employer did not document and utilize specific procedures to control hazardous energy of the equipment such as, but not limited to, the grinder of the PVC Recycle System on Line 1. Employees did not control all hazardous energy sources such as, but not limited to, electrical before performing activities such as, but not limited to, rebuilding the grinder. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work.

The hopper's valves were not isolated from pneumatic energy source prior to engaging in these maintenance activities. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work. B) Nox US, LLC, located at 931 South Springville Ave, Fostoria, OH 44830: On or about August 4, 2017, the employer did not implement energy control procedures on the equipment such as, but not limited to, the grinder of the PVC Recycle System on Line 1 prior to conducting activities such as, but not limited to, rebuilding the grinder.
