29 CFR 1910.1200(f)(6)(i): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information required by 29 CFR 1910.1200(f)(1)(i) through 29 CFR 1910.1200(f)(1)(v).  a.	NOX US, LLC, worksite located at 931 South Springville Avenue, Fostoria, Ohio 44830: On or about July 20, 2023, the employer did not ensure that containers storing hazardous chemicals such as, but not limited to CK UV-705 were labeled and marked with signal words, hazard statements, pictograms, precautionary statements and name address, and telephone number of the chemical manufacturer, in that multiple barrels in the work area were only marked with the product name. b.	NOX US, LLC, worksite located at 93 I South Springville Avenue, Fostoria, Ohio 44830: On or about July 20, 2023, the employer did not ensure that containers storing hazardous chemicals such as, but not limited to CK UV-405 were labeled and marked with signal words, hazard statements, pictograms, precautionary statements and name address, and telephone number of the chemical manufacturer, in that a barrel of CK UV -405 was miss marked with a CK-UV 705 label in the work area and multiple barrels in the work area were only marked with the product name.

29 CFR 1910.132(d)(1)(i): When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employee.  a. NOX US,LLC, worksite located at  931 South Springville Avenue, Fostoria, Ohio 44830: On or about June 30, 2023, the employer did not perform an adequate hazard assessment to determine the proper protective equipment to be worn by employees who utilize Methyl Ethyl Ketone to clean in and around the UV Area in and around the UV Area in Lamination Line l. Employees performing cleaning duties in this condition were exposed to dermal injuries from methyl ethyl ketone or UV chemicals.  NOX US, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.132(d)(1)(i), which was contained in OSHA inspection number 1419279, citation number 1, item number 1a and was affirmed as a final order on January 17, 2022, with respect to a workplace located at 931 S. Springville Ave., Fostoria, Ohio 44830.

29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes.  a. NOX US, LLC, worksite located at 931 South Springville Avenue, Fostoria, Ohio 44830: On or about July 24, 2023, the employer did not ensure that employees who used Methyl Ethyl Ketone utilized adequate personal protective equipment for the hands. Employees that handled and used the chemical to clean rollers and/or the roller knife in and around the UV Area in Lamination Line I were exposed to dermal hazards from methyl ethyl ketone or UV chemicals.  NOX US, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.138(a), which was contained in OSHA inspection number 1419279, citation number 1, item number 1b and was affirmed as a final order on January 17, 2022, with respect to a workplace located at 931 S. Springville Ave., Fostoria, Ohio 44830.
