29 CFR 1910.304(b)(3)(ii)(B): Receptacles other than 125 volt, single-phase, 15-, 20-, and 30-ampere receptacles that are not part of the permanent wiring of the building or structure and that are in use by personnel shall have ground-fault circuit-interrupter protection for personnel. WORKSHOP/CELL No. 6: The black flexible cord of the No. 2 Stake of the No. 27 Air Press, unknown identification, was not plugged into a safely working Ground Fault Circuit Interrupter (GFCI) for appropriate and safe protection for all personnel at all times. On/or about July 24, 2017, the above GFCI had suddenly tripped when both hands of the operator were in the point-of-operation while performing normal tasks, and the upper ram/die landed on both hands of the operator. The operator sustained very severe crushing/smashing/amputation to the fingers on both hands from the incident.

Metal raceways, cable trays, cable armor, cable sheath, enclosures, frames, fittings, and other metal noncurrent-carrying parts that are to serve as grounding conductors, with or without the use of supplementary equipment grounding, shall be effectively bonded where necessary to ensure electrical continuity and the capacity to conduct safely any fault current likely to be imposed on them: WORKSHOP/CELL No. 6: The metal conduit at the No. 2 stake/No. 27 press, unknown identification ,was not permanent, continuous and effective for proper personnel protection.

29 CFR 1910.305(j)(2)(i): All 15- and 20-ampere attachment plugs and connectors shall be constructed so that there are no exposed current-carrying parts except the prongs, blades, or pins. The cover for wire terminations shall be a part that is essential for the operation of an attachment plug or connector (dead-front construction). Attachment plugs shall be installed so that their prongs, blades, or pins are not energized unless inserted into an energized receptacle. No receptacles may be installed so as to require an energized attachment plug as its source of supply. WORKSHOP/CELL No. 6: The attachment plug for the No. 1 stake press, unknown identification was smashed at the receptacle.
