29 CFR  1910.147(c)(4)(i):Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.  On or before May 28, 2024, the employer did not ensure the developed and documented hazardous energy control procedure was utilized for the control of the potentially hazardous electrical, pneumatic, and gravitational energy sources when die-setter employee(s) were engaged in service and maintenance activities of the Federal 45-ton Part-Revolution Mechanical Power Press, known as Press 45F3, and other mechanical power presses serviced at the facility.  Die-setter employee(s) perform service and maintenance tasks without utilizing hazardous energy control procedure(s) to perform tasks, such as cleaning inside the press die space, clearing jams inside the press die space, and die setting operations that require the removal, installation, and adjustment of the machine dies of Press 45F3 and other mechanical power presses serviced at the facility.

29 CFR  1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  On or before May 28, 2024, the employer did not ensure die-setter employees engaging in service and maintenance activities of the Federal 45-ton Part-Revolution Mechanical Power Press, known as Press 45F3, and the other mechanical power presses serviced at the facility applied and followed the established energy control procedure by; turning off or shutting down the press [per 1910.147(d)(2)], isolating the press electrical, pneumatic, and gravitational energy sources [per 1910.147(d)(3)], affixing and/or removing lockout devices [per 1910.147(d)(4)(i)], or verifying de-energization and isolation had been accomplished [per 1910.147(d)(6)], prior to performing service and maintenance activities.  Service and maintenance activities conducted by die-setter employees included, cleaning inside the press die space, clearing jams inside the press die space, and die setting operations that required the removal, installation, and adjustment of the machine dies of Press 45F3 and other mechanical power presses serviced at the facility.

29 CFR  1910.147(c)(4)(ii):The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance:  On or before May 28, 2024, the employer did not ensure the developed and documented hazardous energy control procedure for the Federal 45-ton Part-Revolution Mechanical Power Press, known as Press 45F3, and the other mechanical power presses service at the facility clearly and specifically outlined the scope, purpose, rules, and techniques for the intended use of the procedure to be utilized while employees performed service and maintenance activities, such as cleaning inside the press die space, clearing jams inside the press die space, and press die-setting operations.  The energy control procedure did not clearly identify the specific procedural steps for shutting down, isolating, blocking, and securing the presses electrical, pneumatic, and gravitational energy sources [per 1910.147(c)(4)(ii)(B)].  The energy control procedure did not clearly identify the specific procedural steps for the placement, removal, or transfer of lockout devices, such as personal locks, die plug device, or die ram block device and the responsibility for them [per 1910,147(c)(4)(ii)(C)].

29 CFR  1910.147(c)(6)(i):The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  On or before May 28, 2024, the employer did not ensure a periodic inspection of the energy control procedure for the mechanical power presses, including the Federal 45-ton Part-Revolution Mechanical Power Press, known as Press 45F3, was conducted at least annually to ensure the procedure and requirements of the 1910.147 standard were being followed.  The employer did not conduct a periodic inspection of the energy control procedure to correct any deviations or inadequacies in the procedure [per 1910.147(c)(6)(i)(B)].  The employer did not conduct a periodic inspection that included a review of the authorized die-setter employee's responsibilities under the energy control procedure [per 1910.147(c)(6)(i)(C)].  The employer did not certify that periodic inspections of the energy control procedure had been performed or a periodic inspection of authorized employee's responsibilities under the procedure had been performed [per 1910.147(c)(6)(ii)].

29 CFR  1910.147(c)(7)(i):The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following:  On or before May 28, 2024, the employer did not ensure authorized and affected employees were provided training in the purpose and function of the energy control program to ensure they understood the program and had the knowledge and skills required for the safe application, usage, and removal of energy controls.  The employer did not provide training for die-setter employees authorized to perform die-setting operation, clean press die spaces, and clear jams inside press die spaces in the methods and means necessary for energy isolation and control [per 1910.147(c)(7)(i)(A)].  The employer did not instruct affected employees, such as press operators in the purpose and use of the energy control procedures [per 1910.147(c)(7)(i)(B)].  The employer did not certify that affected employee training for press operators had been accomplished [per 1910.147(c)(7(iv)].
