29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about July 27, 2017 and at times prior thereto, at the worksite located at 4560 Highway 412 East, Siloam Springs, Arkansas, there was inadequate guarding of the area of operations on the following machines: A. 480V CEMCO Multi-press# 1, unknown model and CEMCO Multi-press# 2, unknown model. Press #1 consisted of 8 drill bits and press #2 consisted of 6 drill bits. The drill bits were actuated by a foot pedal. The machine had a front guard that would depress when the foot pedal was actuated; however it would not stop the cycle if an employees hand was in the area of operation. B. Up-cut saws #2 and # 5, unknown model and manufacturer. Both saws were foot actuated and the area of operation was guarded by a stationary guard and employees had the ability to place their entire arm into the area of operation.

29 CFR 1910.303(b)(3): Insulation Integrity. Complete wiring installations were not free from short circuits and from grounds. On or about July 27, 2017 and times prior thereto, at the worksite located at 4560 Highway 412 East, Siloam Springs, Arkansas, the 480V pendant that an employee attempted to use to power a fan was damaged. Both the outer insulation and interior electrical insulation were damaged and there were visible bare wires inside of the cord. Employees were exposed to electrical shock and burns.

Black Jack all weather roof cement, asphalt cement for roofs, one 3 qt. container. E. DAP 100% waterproof window, door and trim sealant, two 5.5 oz. tubes. F. WD-40, 5 12 oz. containers. G. Royal Purple Synthetic Oil, Synfilm recip 100, one qt. H. Lubriguard Dexron III/Mercan automatic transmission fluid, 6 one qt containers. I. Royal Sovereign Asphalt Shingles containing silica, multiple pallets.
