29 CFR 1926.451(b)(3): The front edge of all platforms was not 14 inches (36 cm) or less from the face of the work, and no guardrail systems were erected along the front edge and/or no personal fall arrest systems were used in accordance with paragraph (g) of this section to protect employees from falling:  a) Residential property at 1108 Falling Mill Rd, in Haddon Heights, NJ, 08035, front (North) and side (West) of the building: Employees were exposed to fall hazards of approximately 12 feet where an Alumipole pump jack scaffold was used approximately 32 inches from the face of the building, which occurred on or about 7/13/2017.  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(c)(2)(i): Footings were not level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement:  a) Residential property at 1108 Falling Mill Rd, in Haddon Heights, NJ, 08035, front (North) and side (West) of the building: Employees were exposed to potential scaffold collapse or movement that could result in a fall of approximately 12 feet where an Alumipole pump jack scaffold was not on sound, level, rigid, footings that were capable of supporting the load, which occurred on or about 7/13/2017.  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a) Residential property at 1108 Falling Mill Rd, in Haddon Heights, NJ, 08035, front (North) and side (West) of the building: Employees were exposed to fall hazards of approximately 12 feet where an Alumipole pump jack scaffold was used without a mid-rail screen or an end-stop, which occurred on or about 7/13/2017.   NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
