29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use.  a) Lot 2, The Fields at Blue Barn Meadows, Allentown, PA, Worksite - On or about August 22, 2017, employees engaged in framing work were using a variable reach rough terrain forklift with a job-built scaffold set on the forks as a means of access to the exterior top plate and other elevated working surfaces.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  a) Lot 2, The Fields at Blue Barn Meadows, Allentown, PA, Worksite - On or about August 22, 2017, employees engaged in framing work were using a variable reach rough terrain forklift with a job-built scaffold approximately 18 feet above grade and were not prevented from falling to that lower level by means of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Lot 2, The Fields at Blue Barn Meadows, Allentown, PA, Worksite - on or about Aug 22, 2017, employees were accessing the second floor of a residential home project using extension ladders which were not extended at least 3 feet above the upper landing surface being accessed.
