29 CFR 1910.36(g)(2): An exit access was not at least 28 inches wide at all points:    (a)  On or about 9/20/2017, on the production floor where an employee operates the geo foam machine the exit access  was blocked by sheets of geo foam with a 10 inch access. The employer did not ensure an exit access of 28 inches wide at all points where the operator performed daily work activities.

29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    a) On or about 9/27/2017, an employee working at the grinder placing scrap geo foam in the grinder for recycling was exposed to continuous noise levels at 94.13% of the permissible daily noise exposure or an equivalent sound level of approximately 89.5 dBA (8-hour time-weighted average sound level of 85 dBA) during the 147 minute sampling period; exposure calculations included a zero increment for the 333 minutes not sampled.
