29 CFR 1910.216(b)(1): An accessible safety trip control that operated on contact was not provided in front and/or back of each mill:    a) 1st Floor Grinder Room - On February 8, 2019 and at times prior, the foam grinder had a safety wire around the hopper opening that was inoperative.    Note:  The safety trip control shall be one of the types or a combination thereof as listed in 1910.216(b)(1).

29 CFR 1910.305(a)(2)(iv)(B): Conductors were not run as multiconductor cord or cable assemblies; or installed as permitted in paragraph (a)(2)(i)(C) of this section, and were accessible to unqualified persons:  a) 2nd Floor Foam Storage Area - On February 8, 2019, and at times prior, open wiring was installed between a junction box and two disconnect switches without any conduit or protective shielding.  This exposes the energized cables to damage.
