29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on flat roofs with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, warning line system, or a safety monitoring system. a) Jobsite: The employer did not ensure that employees performing roofing repair operations were protected from falls from the edge of a building by a fall protection system; thus exposing employees to a fall hazards. This condition was observed on/about 14 SEP 2017.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. 1) Jobsite: The employer did not ensure the Werner extension ladder was extended at least 3 feet above the landing to ensure proper mounting and dismounting by employees thus exposing them to a fall hazard. This condition was observed/noted on/about 14 SEP 2017.
