29 CFR 1926.102(a)(2): Eye and face protection equipment did not meet the requirements of ANSI Z87.1-1968, Practice for Occupational and Educational Eye and Face Protection:     a) New residential construction located at 2295 Melina Place, Alpharetta, GA 30009, West side: On or about October 11, 2017, and at times prior, the employer exposed employees to struck-by hazards while operating pneumatic nail guns at 120 PSI, in that ANSI Z87.1-1968 safety glasses were not provided to employees installing siding to the exterior of a structure.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person.    a) New residential construction located at 2295 Melina Place, Alpharetta, GA 30009, West side: On or about October 11, 2017, and at times prior, employees were exposed to fall hazards of over 19-feet, 3-inches without a fall protection system, not provided with proper PPE for concrete fiber board installation, in that the employer did not perform frequent and regular inspections of the project and/or the employees making sure the siding installers were utilizing their fall protection systems.    Bett Mart Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.20(b)(2), which was contained in OSHA inspection number 1225096, citation number 1, item number 1 and was affirmed as a final order on 05/12/2017, with respect to a workplace located at 4415 Atwood Drive, Lot #144, Cumming, GA.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:    a) New residential construction located at 2295 Melina Place, Alpharetta, GA 30009, West side: On or about October 11, 2017, and at times prior, the employer exposed employees to fall hazards of 19-feet, 3-inches from the lower level, in that personal fall arrest systems or guardrails were not provided on the pump jack scaffold system being used by siding installers.     Bett Mart Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 1225096, citation number 1, item number 2 and was affirmed as a final order on 05/12/2017, with respect to a workplace located at 4415 Atwood Drive, Lot #144, Cumming, GA.
