Employees performing roller cleaning on the Littell LM-1E Coil Cutting Line, serial no. 89159-88, on the leveler machine without the benefit of energy isolation, hazardous energy recognition training, and employer evaluation were exposed to caught in and crushing hazards in the event of un-expectant roller start up.     b) Lakeside Metals, Inc. dba TPC Metals LLC. located at 1 Toy Street in Bryan, Ohio; On or about October 12, 2017, the employer did not ensure an energy control program consisting of machine specific control procedures, employee training, and periodic inspections were developed and implemented. Employees engaged in a dye change on the Littell LM-1E Coil Cutting Line, serial no. 89159-88, without the benefit of energy isolation, hazardous energy recognition training, and employer evaluation were exposed to amputation hazards in the event of un-expectant of the press.       Lakeside Metals, Inc. dba TPC Metals, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(1) which was contained in OSHA inspection number 1135772, citation number 1, item number 6 and was affirmed as a final order on May 31, 2016 with respect to a workplace located at 1401 Raff Road SW in Canton, Ohio 44710.

An employee suffered an amputation to the tip of their middle finger on their right hand on October 6, 2017 after their hand was caught in an in-going nip point created on the steel rollers of the Littell Leveler Machine.  The OSHA area office was notified of the amputation on October 10, 2017.
