29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a.  On or about October 10, 2017, at the west side of the building, two employees removing old roof shingles on the first level roof were not wearing hard hats while other employees did roof work above them, exposing to being struck by falling objects.

29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a. On or about October 10, 2017, at the north side of the residential building, an employee using a pneumatic nail gun to install weather resistant material on a 6:12 pitch roof was not wearing eye protection and exposed to flying debris and struck-by hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a.  At the job site, employees working at elevations of 6 feet or more above ground are not protected from fall hazards by the use of a personal fall arrest system.  The employer fails to assure effective protection against falls as required by the standard, as demonstrated on October 10, 2017, when employees doing roof installation work on a 6:12 pitch residential home at 59 19th Ave. North, Jacksonville Beach, Florida 32250 without fall protection were exposed to fall hazards of 30 ft. and 9ft-8in from the leading edge of the building.  Top Gun Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1130987, citation 01, item 01, issued on May 6, 2016, with respect to a workplace at 52 Lagoon Course, Ponte Vedra, FL 32082.  The final order date was June 1, 2016.    Top Gun Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 954790, citation 01, item 02, issued on January 16, 2014, with respect to a workplace at 3722 Crossview Drive, Jacksonville, FL 32224.  The final order date was January 30, 2014.
