29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a. The employer fails to assure effective protection against falls as required by the standard, as demonstrated on March 13, 2018, when employees are working without fall protection on the 5:12 pitch roof, exposing employees to a 17 feet fall hazard.   Top Gun Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1270009, citation number 2, item number 2, and was affirmed as a final order on November 21, 2017, with respect to a workplace located at 59 19th Ave North, Jacksonville Beach, Florida 32250.   Top Gun Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13)), which was contained in OSHA inspection number 1130987, citation number 1, item number 1, and was affirmed as a final order on June 1, 2016, with respect to a workplace located at 52 Lagoon Course, Ponte Vedra, Florida 32082.   Top Gun Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13)), which was contained in OSHA inspection number 1130987, citation number 1, item number 1, and was affirmed as a final order on January 30, 2014, with respect to a workplace located at 3722 Crossview Drive, Jacksonville, Florida 32224.
