29 CFR 1910.178(l)(3)(ii)(G): Operator training did not include workplace related topics such as ramps and other sloped surfaces that could affect the vehicle's stability:    The employer is failing to protect employees from struck-by hazards. This was most recently documented at a worksite located at or near 108 Apollo St., Alda, Nebraska 68810 on or about November 1, 2017. The employer did not ensure that an employee who was using the RC 3000 Series stand-up forklift was given instruction on operation of the forklift on the grade of the slope they were operating the standup forklift on prior to use. The stand-up forklift restricts the safe operation of the forklift on a grade not to exceed 10%. The forklift was operated on a graded slope that exceeded 10% and an employee was injured when the stand-up forklift went down that slope and struck a guardrail that pinned the employee between the guardrail and the stand-up forklift.
