Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:  (a) Core Room - While troubleshooting mechanical problems with Redford 22 core making machines, set-up employees removed/opened fixed guarding that normally enclosed moving machine parts without applying lockout/tagout, exposing these employees to getting caught in or caught by moving machine parts, as happened on or about October 30, 2017, when an employee put his right hand on top of the moving sand magazine and suffered a fingertip amputation after the vertical clamp closed on the sand magazine when the magazine reached the blow position.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
