29 CFR 1910.147(c)(4)(ii)(B): The energy control procedure did not clearly and specifically specify procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:  a) Date of Violation: On or about 11-20-2017 at 9:00 a.m. and times prior thereto, located at DOD Defense Agency Commissary (DeCA Norfolk), 1588 Mall Drive Norfolk, VA 23511 in the Meat Department, Condition: Employee(s) were exposed to amputation hazards while operating a Hollymatic Model 120 Precision Meat Bulker machine with conveyor belt.  Employee(s) were required to open the bulker where product entered the machine to remove excess build-up of meat/fat content between the different productions of quality for meat/fat percentages.  While pulling meat/fat build-up out of the rear of the machine, the guillotine style cutting blade activated subsequently amputating the employees left middle finger tip.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points:  Date of Violation: On or about 11-21-2017 and times prior thereto, located at DOD Defense Agency Commissary (DeCA Norfolk), 1588 Mall Drive Norfolk, VA 23511 in the Meat Department,   a)  Employee(s) were exposed to electrical shock and fire hazards from an electrical power cord with plug coming from the overhead area to provide power at and near the meat bulker machines.  The cord was wrapped around metal shelving brackets and was not protected from sharp edges.  The electrical cord had visible damage and cuts to the outer jacket.  b)  Employee(s) were exposed to electrical shock and fire hazards from an electrical power cord with plug coming from the overhead area to provide power to the meat department equipment at or near the product meat tray racks.  The cord was wrapped around metal shelving brackets and was not protected from sharp edges.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  Date of Violation: On or about 11-21-2017 and times prior thereto, located at DOD Defense Agency Commissary (DeCA Norfolk), 1588 Mall Drive Norfolk, VA 23511 in the Meat Department:  a) Employee(s) were exposed to an electrical shock hazard from an electrical power cord used to power the meat bulker machines with a plug end that had inadequate strain relief.  b) Employee(s) were exposed to an electrical shock hazard from an electrical power cord with plug end that had inadequate strain relief.  The cord was located in the meat bulker machine area near the meat flat tray product racks.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

The installed guarding device in place was not used to prevent the operator from having any part of their body in the danger zone(s) during the operating cycle(s).   (a) Date of Violation: On or about 11-21-2017 and times prior thereto, located at DOD Defense Agency Commissary (DeCA Norfolk), 1588 Mall Drive Norfolk, VA 23511 in the Meat Department, Condition: Employee(s) were exposed to amputation hazards while cutting meat using a Hobart Vertical Band Saw, DeCA Property tagged #0020440 with unused portions of the saw blade exposed and not guarded.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
