Employee(s) were required to open the bulker where product entered the machine to remove excess build-up of meat/fat content between the different productions of quality for meat/fat percentages.  While pulling meat/fat build-up out of the rear of the machine, the guillotine style cutting blade activated subsequently amputating the employees left middle finger tip.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

The cord was wrapped around metal shelving brackets and was not protected from sharp edges.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
