OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck by hazard:  a) Reynen J Well Pad, New Milford, PA: A swage lifting cap specifically designed for lifting the swage fitting during installation and removal was used to lift a landing joint (a thirty-eight (38) foot piece of casing pipe (13 3/8 inch diameter/~2,000 lbs). As the three-worker crew attempted the lift to lay the pipe down, the lifting cap failed, causing the pipe to fall, striking the crew, on or about January 4, 2018.   ABATEMENT CERTIFICATION REQUIRED
