(5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential crushing hazards from falling loads and/or lift parts:  (a) On or about February 1, 2018 for the employees exposed to struck-by hazards where the service rigs fast line load exceeded the manufacturer's recommendation of 41,667 pounds and the draw works brake systems did not fully function causing a traveling block and bailers to crash to the operators platform of a WISCO Servicing Rig at the well site identified as Johnson 24-1-2H in the vicinity of Williston, ND.  Abatement Note: Among other methods, one feasible and acceptable method of abatement to correct this hazard would be for the employer to follow the manufacturers' recommendations for lifting with a single or double line and performing required maintenance and inspection of brake systems and brake safety systems.
