Section (5)(a)(1): The employer failed to furnish to each of his employees employment and a place of employment which was free from recognized hazards that were causing or are likely to cause death or serious physical harm, in that employees were exposed to fire hazards:  (a)   On or about November 30, 2018 for employees who were exposed to a flash fire while starting the rig air compressor when flammable anti-freeze was expelled and ignited from a methanol pot that was heated by a weed burner at the facility located at Federal 14-7H Wellsite in Trotters, North Dakota.  Abatement note: Among other feasible and useful means of abatement, the employer can follow Service King manufacturers recommendation to drain the air compressor tank daily to prevent the ice blockage or to follow the CRC Air Brake Anti- Freeze & Conditioner instruction, and thaw ice blockage in the methanol pot at room temperature.
