29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval.    On or about 5 March 2018, and at times prior thereto, job-made wooden trash boxes were being used to remove construction debris from the second story roof of the Main Building, without the approval of the forklift manufacturer, exposing the operator and those nearby to the hazard of being struck by the trash box and the contents.  1) Small Trash Box: This box was built by the General Contractor and used once before for approximately 30 minutes.  2) Large Trash Box: This box was built and used by the employees for approximately 3 hours on the day of the inspection.
