29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury:  (a) On or about June 24, 2023, employees performing the work of cut outs on a precast concrete wall were not protected from a crushed-by hazard in that no written work instructions had been developed to address securing the block so that it will only fall in the direction intended prior to commencement of pushing.
