This exposed employees to a fire, explosion, or inhalation hazards due to an ammonia release.        b) On or about March 1, 2018, at the Lone Star Bakery in China Grove, TX, the employer did not document that equipment in the process complied with recognized and generally accepted good engineering practices (RAGAGEP) such as ASME Boiler and Pressure and Vessel Code Section VIII UG-116; when the employer failed to ensure that the horizontal re-circulator HLR-2 had a nameplate or U-1 documentation.
