29 CFR 1910.119(d)(3)(ii):The employer did not document that equipment complies with recognized and generally accepted good engineering practices: Instance a) On or about October 24, 2023, in the Plant 2 Refrigeration Engine Room at Lone Star Bakery located in China Grove, Texas; employees were exposed to fire, explosion and toxic gas release hazards when the employer did not replace or recertify the pressure relief valves every five years according to their adopted RAGAGEP, IIAR Bulletin 110 (Start-up, Inspection and Maintenance of Ammonia Mechanical Refrigerating Systems) Edition 1993, Section 6.6.3. Instance b) On or about October 24, 2023, in the Plant 1 and Plant 2 Refrigeration Engine Rooms at Lone Star Bakery located in China Grove, Texas, employees were exposed to fire, explosion and toxic gas release hazards when the employer did not ensure the ammonia refrigeration piping and system components; including but not limited to the King Valve (HPR1-19) on the High Pressure Receiver was properly identified according to their adopted RAGAGEP, IIAR Bulletin No. 109 (Minimum Safety Criteria for a Safe Ammonia Refrigeration System) Edition 1997 , Section 4.10.3.
