29 CFR 1926.20(b)(1): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.    a. On or about March 15, 2018, located at Lot 1775 at 912 Admiral Drive in Sunbury, Ohio, employees were installing siding on a residential home while working from a steep-pitched roof, chicken ladders and pump jack scaffolding without the employer initiating and maintaining safety and health programs, thereby exposing the employees to fall and struck-by hazards.

29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.      a. On or about March 15, 2018, located at Lot 1775 at 912 Admiral Drive in Sunbury, Ohio, employees were installing siding on a residential home while working from a steep-pitched roof, chicken ladders and pump jack scaffolding without the employer instructing the employees to recognize and avoid the hazards associated with falls and struck-by hazards located on the site.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.   a. On or about March 15, 2018, located at Lot 1775 at 912 Admiral Drive in Sunbury, Ohio, employees were installing vinyl siding while on a pump jack scaffold without any means of fall protection, thereby exposing the employees to a fall hazard of 11 feet.
