29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section:     On or about 21 March 2018, the employer did not document hazardous energy control procedures for use by employees assigned to perform servicing or maintenance tasks on the 14 CNC Mills and four CNC Lathes. Employees were exposed to unexpected start-up (energizing) likely to cause fractures and other injuries associated with caught-in and struck-by hazards.    Abatement documentation is not required for this item.

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed:    On or about 21 March 2018, the employer did not conduct a periodic inspection of the energy control procedures at least annually to ensure that the procedures and the requirement of this standard were being followed. Employees utilizing undocumented procedures were exposed to caught-in hazards and struck-by hazards.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     On or about 21 March 2018, the employer failed to provide machine guarding to protect operator(s) and other employees from hazard(s) created by production equipment, including 14 CNC Mills and four CNC Lathes. The employer removed or bypassed all of the safety (interlock) switches on the main access doors of 18 machines. This exposed employees operating the equipment to caught-in, crushing, and struck-by hazards while feeding, removing and working near the parts being machined.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.215(a)(4): Work rests. On offhand grinding machines, work rests shall be used to support the work. They shall be of rigid construction and designed to be adjustable to compensate for wheel wear. Work rests shall be kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage:    On or about 21 March 2018, the employer failed to ensure that the work rests were attached on the Bradford Craftsman Grinders. This exposed employees to struck-by hazards from wheel breakage likely to cause lacerations.    Abatement documentation is not required for this item.
