29 CFR 1910.22(d)(2): Hazardous conditions were not immediately repaired or properly guarded before an employee used the walking-working surface.   2331 Mellon Court, Decatur, Georgia- On or about 3/23/18, and times prior to, employees working in and around the waste water/milling area, were exposed to a tripping hazard due to a hole in the floor of the concrete.  The damaged concrete created a hazard for pedestrian traffic as well as forklift operations.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways or wherever turns or passage had to be made:  On or about 3/23/28, and times prior to, in the BB1 and BB2 dispensing areas, employees were exposed to chemical spill hazards when drums of chemicals are not protected from forklift traffic.  Drums of propylene glycol, defoamer, resins, additives, etc. were subject to being struck by forklifts while transporting product in and round the dispensing areas.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
