29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization(n, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  a.  2331 Mellon Court, Decatur, GA (Line #18 Injection Paint Filler Machine) - On or about 12/10/19,  Employees were exposed to sudden energization or start-up of multi-energy source equipment/machinery such as, but not limited to line  18 injection filler machines.  The company's written lockout/tagout specific procedures were found with the following deficiency:                                                                                                                                                                                                                                                                                               a.  The energy control procedures did not specifically outline the steps for the removal of            locks and restoring the equipment back to service.

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed:     a.     2331 Mellon Court, Decatur, GA (Line #18 Injection Paint Filler Machine) - On or about 12/10/19 - Employees were exposed to sudden energization or start-up of multi-energy source machines.    The employer did not conduct periodic inspections of the Lockout/Tagout procedures to assure that the procedures were being followed and corrections were made as needed to the Injection Paint Filler Machines and other multi-energy source machines.

29 CFR 1910.212(a) (1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a.  2331 Mellon Court, Decatur, GA (Line #18 Injection Paint Filler Machine) - On or about 12/10/19, Employees were exposed to pinch point hazard. The employees are required to work in close proximity to the pneumatic paint moving cylinders, while making adjustments, such as, slowing the paint line down. Employees can accidentally put their hands where the moving pneumatic cylinders/pistons moves up and down where the adjustments/work is being performed.

29 CFR 1904.31(a): 29 CFR 1904.31(a): The employer did not record the recordable injuries/illnesses that occurred to employees who were not on their payroll, but were supervised by the company on a day-to-day basis:  a.   2331 Mellon Court, Decatur, GA (Throughout Facility) - On or about 12/10/19, the employer (host) is not recording injuries/illnesses of the temporary workers onto their OSHA-300 logs, per the temporary service agreement. The host employer (Plaid Enterprises, Inc.) supervised the temporary workers on a daily basis.
