29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:    Production Area - On or about March 24, 2018 and before the employer did not train an employee on lockout/tagout in order for the employee to safely perform work on machines, including die changes/set-up. The employee did not acquire the requisite knowledge and skills for the safe application, usage, and removal of the energy controls and was exposed to caught in hazards from pinch point(s).    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.217(e)(3): It shall be the responsibility of the employer to insure the original and continuing competence of personnel caring for, inspecting, and maintaining power presses:    On or about March 24, 2018 and before, the employer did not ensure that employee(s) assigned to independently care for and maintain mechanical power presses had gained the original competence to safely accomplish such task(s).     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.217(d)(9)(iv): The employer shall provide and enforce the use of safety blocks for use whenever dies are being adjusted or repaired in the press:    Production Area - On or about March 24, 2018, employee(s) were not required to use safety blocks to keep the dies apart while making repairs or adjustments in the point of operation of mechanical power presses, including the Minster #465. Employee(s) were exposed to being caught in between the pinch point created by the die set.      Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
