29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about April 10, 2018, energy control procedure were not documented nor utilized for inspection and maintenance tasks on Rexroth TS-5 gearbox(es). Employees assigned to perform such tasks were exposed to caught-in hazards likely to cause amputation. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about April 10, 2018 an employee was performing servicing and maintenance work on a gearbox for the "Front Brake Line" conveyor line and failed to perform the following actions to isolate the hazardous energy sources. a) turn off and shut down the machine. b) physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s). c) affix lockout or tagout devices to each energy isolating device. d) verify that isolation and de-energization of the machines or equipment have been accomplished. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.333(c)(10): Only a qualified person following the requirements of paragraph (c) of this section may defeat an electrical safety interlock, and then only temporarily while he or she is working on the equipment. The interlock system shall be returned to its operable condition when this work is completed: On or about April 10, 2018 the supervisor defeated the safety interlock on an energized 480 volt "front brake line" cabinet to gain access to motor control circuitry and switch off a circuit breaker to perform work on a gearbox. Employee(s) were not qualified nor protected from electrical hazards, such as arc flash, while turning off the breaker inside of the energized the cabinet. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed: On or about April 10, 2018 employees were not required to wear appropriate personal protective equipment while exposed to switching circuit breakers near exposed live parts operating at 480 Volts to turn power off to one of multiple motors of a conveyor line. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
