Piston Automotive, LLC located in Toledo, OH:  On or about October 9, 2018, the employer did not ensure employees locked out the S40/50 LRO cell prior to working on the drive motor and pulleys for the primary lift.  Employees were exposed to numerous crushing hazards when working directly adjacent to moving axle carriages and they were exposed to crushing hazards from the potential start up of the primary lift.  b. Piston Automotive, LLC located in Toledo, OH:  On or about October 22, 2018, the employer did not ensure employees and supervisors locked out the S40/50 LRO cell prior to entering the cell to adjust sensing cables on the axle carriages.  Employees were exposed to crushing hazards from the unexpected start up of the primary carriage.  c.Piston Automotive, LLC located in Toledo, OH:  On or about October 22, 2018, the employer did not ensure employees and locked out the S200 front axle packoff robot before entering the work cell to perform maintenance on the robot gripper.  Employees were exposed to struck-by and crushing hazards from the robot.
