Piston Automotive, LLC located in Toledo, OH: On or about October 22, 2018, the employer did not ensure a lockout device used by a maintenance employee, indicated the employees identity. The lockout device was being used by the employee to lockout the pack off robot on the rear axle line.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program was understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. a. Piston Automotive, LLC located in Toledo, OH: On or about October 22, 2018, the employer did not ensure employees were trained on the function of the company's lockout program and the program was understood by employees. Employees were not required to lock out the carriages for the S40/50 LRO line when adjusting sensing cables and employees were not required to lockout the robot gripper head when making adjustments to the locating pins.

29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the following elements and actions and shall be done in the following sequence. a. Piston Automotive, LLC located in Toledo, OH: On or about October 9, 2018, the employer did not ensure employees locked out the S40/50 LRO cell prior to working on the drive motor and pulleys for the primary lift. Employees were exposed to numerous crushing hazards when working directly adjacent to moving axle carriages and they were exposed to crushing hazards from the potential start up of the primary lift. b. Piston Automotive, LLC located in Toledo, OH: On or about October 22, 2018, the employer did not ensure employees and supervisors locked out the S40/50 LRO cell prior to entering the cell to adjust sensing cables on the axle carriages. Employees were exposed to crushing hazards from the unexpected start up of the primary carriage. c.Piston Automotive, LLC located in Toledo, OH: On or about October 22, 2018, the employer did not ensure employees and locked out the S200 front axle packoff robot before entering the work cell to perform maintenance on the robot gripper. Employees were exposed to struck-by and crushing hazards from the robot.

Employees would need to open the electrical disconnect and expose themselves to energized conductors to deenergize the equipment.
