29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces were inspected, regularly and as necessary, and maintained in a safe condition: a) Tapping area furnace #2: On or about April 24, 2018, an employee(s) moving ladles from the discharge area of furnace #2 while on a powered industrial truck that is used to move the ladle with the molten Silicon are exposed the burn hazards due to irregularities in the surface it in required to travel over.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) Tapping area furnace #2: On or about April 24, 2018, the employer exposed employees operating the un-enclosed A&G Mercury tug to struck-by and burn hazards by allowing employees to operate the tug while pulling ladles/crucibles containing molten Silicon at 2600 degrees Fahrenheit that can overflow the top of the ladles/crucibles and cause an explosion without the personal protective equipment to protect the employees face, neck and extremities.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): a) Tapping area furnace #2: On or about April 24, 2018, the employer exposed employees operating the un-enclosed A&G Mercury tug to struck-by and burn hazards by allowing employees to operate the tug while pulling ladles/crucibles containing molten Silicon at 2600 degrees Fahrenheit without assessing the need for personal protective equipment (sides and back of head were not protected by the PPE provided).
