29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.  a) At the site, on or about May 2, 2018 employees were using an anchor point that was not installed according to the manufacturer's instruction, in that roof underlayment nails were used. The employees were exposed to a fall height of approximately 9 feet 4 inches.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) For employees working on the roof, on or about May 2, 2018, who were exposed to a fall height of approximately 9 feet 4 inches while conducting roofing work, in that, fall protection was not used. The slope of the roof was 5/12.     Gulf Western Roofing & Sheet Metal, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1169793, citation number 1, item number 1 and was affirmed as a final order on 12/22/2017, with respect to a workplace located at 8130 Chilton Drive, Orlando, FL 32835.
