29 CFR 1926.1419(j): Anyone who became aware of a safety problem did not alert the operator or signal person by giving the stop or emergency stop signal:    a) On or about 4/20/2018; two employees were rigging while standing on top of a form table which was partially suspended by a tower crane and approximately two thirds out of the fifteenth floor of the building; when the table began to move unexpectedly out of the building, the flagger/signal person did not give the stop or emergency stop signal to the tower crane operator exposing the employees working on top of the table to a struck-against hazard.
