29 CFR 1926.21(b)(2): The employer did not instruct each employee in the avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards: This violation occurred on or about August 9, 2018, where two employees working close to the top of an approximately twenty-foot tall free-standing shoring tower during dismantling operations and lowering a section of shoring secured to the tower by a rope, were exposed to a tower tip-over hazard.
