The employer fails to assure effective protections against falls as required by the standard, as demonstrated on May 24, 2018, when a male employee wearing a red hat, blue shirt, and blue jeans, is working on a residential roof with a 5:12 pitch at 1116 Nochaway Drive, Saint Augustine, Florida, and is exposed to a 13 foot fall hazard to the ground in that he is not attached to a personal fall protection system.  The employer fails to assure effective protections against falls as required by the standard, as demonstrated on May 24, 2018, when a male employee wearing a straw hat, blue shirt, and blue jeans, is working on a residential roof with a 5:12 pitch at 1116 Nochaway Drive, Saint Augustine, Florida, and is exposed to a fall hazard of 13 feet to the ground in that he is not attached to a personal fall protection system.  The employer fails to assure effective protections against falls as required by the standard, as demonstrated on May 24, 2018, when a male employee wearing a green sprite box as a hat, white and maroon shirt, and red pants, is working on a residential roof with a 5:12 pitch at 1116 Nochaway Drive, Saint Augustine, Florida, and exposed to a fall hazard of 13 feet to the ground in that he is not attached to a personal fall protection system.  The employer fails to assure effective protections against falls as required by the standard, as demonstrated on May 24, 2018, when a male employee wearing a round dark hat, plaid shirt, and jeans, is working on a residential roof with a 5:12 pitch at 1116 Nochaway Drive, Saint Augustine, Florida, and is exposed to a fall hazard of 13 feet to the ground in that he is not attached to a personal fall protection system.
